    Volunteer Criminal Background Form-8120F1

     Form 8120 F1 is used by school volunteers to grant permission to the School Corporation to run a criminal background check prior to volunteering in the schools.

    Parents should complete the form and return it to their child's school. This should be done prior to volunteering in the schools or chaperoning a field trip.

    If you live in any of the following situations:

    In a Shelter

    In a Motel or campground due to lack of accommodation

    In a car, park, abandoned building or bus/train station

    Doubled up with other people due to loss of housing or economic hardship

    You may qualify for certain rights and protections under the federal McKinney-Vento Act.
