How to obtain a Work Permit
- Every employed student between the age of 14 and 18 must have a work permit. In order to obtain a work permit, you must have an "Intent to Employ Form" completed and signed by the employer and parent/guardian. If the employer does not provide you with the "Intent to Employ Form" you may pick up a blank copy in the CPHS Main Office.
- You should bring the completed "Intent to Employ Form" to the Main Office Secretary to obtain the work permit. (Allow 24-48 hours for processing.) Typically, the "Intent to Employ Form" should be dropped off before school and picked up after school.
- Any child between the age of 14 and 18 who lives in the Crown Point Community School Corporation District may come to the CPHS Main Office and request a work permit be processed. Students who do not attend Crown Point Schools will need to provide a "birth certificate" with the "intent to employ form" to receive a work permit.
