Registration for New Students

Description

New students enrolling must be registered in the Timothy Ball Office.

Please bring the following documents with you to register your student:

Student's Birth Certificate, from the Health Dept. (NOT the Hospital Certificate)
Student's Social Security Card
Student's Immunization Record
Parent's Driver's License
3 Proofs of Residency - Nipsco, Water Bill, copy of Lease (we cannot accept phone bills). If the parent is living with a relative, the parent & relative must go to Central Office, see John Williams/Sheri Anderson & fill out a Third Party Affidavit before enrolling.
