6th Grade Physical Exam Cards
 
The nurse's office is currently trying to document all 6th grade physical exam cards. They are yellow, 5"x8" cards with the words "Crown Point Community School Corporation" on the front side. All 6th graders attending Crown Point Schools during 5th grade should have received this card from their school nurse near the end of the school year. The Crown Point School Corporation's policy is for all 6th and 9th graders to have a physical on file.
 
If your child still has the completed card, please have it brought to the nurse's office as soon as possible. If your child never received a yellow card or it is lost, please contact the nurse's office at Wheeler. There are faxable forms available to send with both sides of the physical card. Please have the card filled out if a physical was done this past summer and the form is not located. Your student may bring it in directly to the nurse's office or mail or fax it to the nurse's attention. If a physical has not been completed, please call the nurse's office if an appointment is made and when it will arrive.
These physicals help the nurses as well as other school personnel who need to know by informing them of the health and development of your child during their school years. Their bodies change a lot during adolescence and sometimes new problems are found like diabetes or scoliosis. Also, no student in the Crown Point Community School Corporation will be allowed to participate in school athletics without a yearly physical on record releasing him or her for physical activity.
