How to obtain a Work Permit

1.  Every employed student between the age of 14 and 18 must have a work permit. In order to obtain a work permit, you must have an "Intent to Employ Form" completed and signed by the employer and parent/guardian.  If the employer does not provide you with the "Intent to Employ Form" you may pick up a blank copy in the Guidance Office.

2.  You should bring the completed "Intent to Employ Form" to the guidance office secretary to obtain the work permit.  (Allow 24-48 hours for processing.)  
3.  Any child between the age of 14 and 18 who lives in the Crown Point Community School Corporation District may come to the CPHS Guidance Office and request a work permit be processed.  Students who do not attend Crown Point Schools will need to provide a "birth certificate" with the "intent to employ form" to receive a work permit. 
Notice:  The back of the form has the hours a student is allowed to work depending on their age.  You will need a different work permit for each employer if you have more than one job or change jobs.
