How to obtain a Work Permit
1. Every employed student between the age of 14 and 18 must have a work permit. In order to obtain a work permit, you must have an "Intent to Employ Form" completed and signed by the employer and parent/guardian. If the employer does not provide you with the "Intent to Employ Form" you may pick up a blank copy in the Guidance Office. |
2. You should bring the completed "Intent to Employ Form" to the guidance office secretary to obtain the work permit. (Allow 24-48 hours for processing.) |
3. Any child between the age of 14 and 18 who lives in the Crown Point Community School Corporation District may come to the CPHS Guidance Office and request a work permit be processed. Students who do not attend Crown Point Schools will need to provide a "birth certificate" with the "intent to employ form" to receive a work permit. |
Notice: The back of the form has the hours a student is allowed to work depending on their age. You will need a different work permit for each employer if you have more than one job or change jobs. |
