Required immunizationsare 5 doses of DPT (Diphtheria, Pertussis, Tetanus); 4 doses polio, 2 doses MMR (mumps, measles rubella),3 doses of Hepatitis B vaccine and one dose of the varicella (chicken pox) vaccine. Students who have had the disease need a written statement from the parent/guardian indicating dates of disease and signed by a parent. Documented evidence from a physician, including dates of immunization, must be presented by the first day of classes.
